On the myProvidence.org website, you and your employees will find a number of tools that support good health management.

We understand that health care is very personal. That's why many of the Health Balance programs are highly personalized, such as our Health Risk Assessment tool and the Personal Health Record. And with all Health Balance resources employee privacy and confidentiality are assured.

Of course, the first step to becoming healthier is knowing what needs improvement.

The Health Risk Assessment tool will help your employees identify and address areas where they can improve their health and their health habits.

The Health Risk Assessment is a self-evaluation tool that includes an individually tailored, holistic health assessment, a personalized report and a motivational action plan.

These resources may include referrals to appropriate online condition or lifestyle centers based on an individual's health risk. And your employees will receive ongoing advice and encouragement to help them along the way.

Providence Health Plan reaches out to employees with chronic health problems and to employees who are at high risk of disease. Your employees may choose to share information from their Health Risk Assessment with Providence. Those that do will be screened to identify which health education and disease management programs might benefit them.

The at-risk employee will receive prevention and health management tools, encouragement and ongoing support by phone and by mail from Providence health educators. Privacy is assured. No one else will be notified of their condition.

Another valuable online tool is the Personal Health Record, which allows your employees to securely gather, store, and manage their individual and family's health information. Once created, it’s right there when they want it and will be kept completely confidential.
